Chronic conditions don't follow appointment schedules. For providers managing diabetes, hypertension, and complex comorbidities, the most important health events happen between visits — and traditional care models are blind to them. Caregenix was founded to close that gap with continuous, technology-enabled care.
AwaitSol engineered the CareGenix Portal: the platform through which providers monitor patients remotely, manage personalized care plans, and run structured care-management programs day to day.
The Challenge
Remote care generates relentless data — vitals streaming from blood-pressure cuffs, glucometers, scales, and wearables across an entire patient panel. Raw streams are useless to a busy care team; the platform had to distill them into a triaged view that answers one question: which patient needs attention right now?
The structured care programs Caregenix runs — Chronic Care Management (CCM), Behavioral Care Management (BCM), and Remote Patient Monitoring (RPM) — add a second layer of difficulty. Each is billing-coded work with specific requirements for time tracking, documentation, and patient interaction. Generic tools can't represent those workflows, and getting them wrong has direct revenue and compliance consequences for providers.
Our Approach
We mapped the clinical workflows first — how care managers actually move through a day of patient monitoring, outreach, and documentation — and shaped the portal around that reality rather than around the data model. The result is a triage-first interface: anomalies and overdue tasks surface to the top, routine stability stays out of the way.
Device data flows through a real-time pipeline that normalizes readings from heterogeneous hardware into unified patient timelines, with configurable thresholds that trigger alerts when a patient's trend demands intervention.
CCM, BCM, and RPM requirements were built in as first-class workflows: time spent on each patient is captured as care managers work, documentation accumulates against program requirements automatically, and audit trails record every clinical action — so compliance is a by-product of using the platform, not a separate chore.
